Emergency / Medical Travel (Residents of Qatar)
If you are living and working in Qatar traveling to the United States for medical purposes you are on the right page.
Prepare
- Photo: Recent color passport size with white background and one electronic photo for your online application. (JPEG file format / 240 KB or less) REMINDER: No head covers except for religious purposes.
- Pay your fee at Commercial Bank of Qatar to receive the yellow receipt for each application. REMINDER: Fees are not refundable and they are not transferrable.

- One application for each person traveling (also for each child): https://ceac.state.gov/genniv/ (Please print confirmation page.)
- Passport
- (Medical Travel only) A letter from a physician or medical facility in the U.S. stating all of the following:
- Arrangements for treatment has been made
- The date the treatment will begin
- The estimated duration of treatment
- The estimated total cost of treatment
- (Emergency Travel Only)
- An official letter from the appropriate facility listing your name and the patient’s information and condition, and the contact information of the attending physician and facility in the United States.

- Copy of recent Death Certificate of close family member.
- Expired passports (if you have them)
- Dual nationals, please bring both passports (if this applies to you)
Attend Your Interview:
{Emergency / Medical Travelers may attend an interview without an appointment, Sunday through Thursdays from 8 – 10 am, except official holidays}

REMEMBER:
Personal appearance is required by applicants 14 years and older on the day of their scheduled appointment. Please arrive at your scheduled time. To speed up the process for entry, only applicants are required to come.
