Visa Forms

1)  DS-156 application for non-immigrant (temporary stay) visa

Every applicant for a non-immigrant visa, including children, must present a separate, completed Form DS-156  at the time of the interview at the U.S. Embassy.  This form requests your basic biographical data (name, passport number, place anddate of birth, gender, contact info, etc.), Information about your trip, and certain questions about you.  You will also attach your photograph to this form. 

This form must be completed online, at http://evisaforms.state.gov

2)  DS-157 Supplemental Nonimmigrant Visa Application

All non-immigrant visa applicants over the age of 16 must present a completed Form DS-157 in addition to Form DS-156. The DS-157 asks additional questions about your previous travel, if you have served in a military, any educational institutions you’ve attended, and similar questions.
