Visa Forms

1)  DS-156 application for non-immigrant (temporary stay) visa

Every applicant for a non-immigrant visa, including children, must present a separate, completed Form DS-156  at the time of the interview at the U.S. Embassy.  This form requests your basic biographical data (name, passport number, place anddate of birth, gender, contact info, etc.), Information about your trip, and certain questions about you.  You will also attach your photograph to this form. 

This form must be completed online, at http://evisaforms.state.gov

2)  DS-157 Supplemental Nonimmigrant Visa Application

All non-immigrant visa applicants over the age of 16 must present a completed Form DS-157 in addition to Form DS-156. The DS-157 asks additional questions about your previous travel, if you have served in a military, any educational institutions you’ve attended, and similar questions.

3)  Additional Forms for Students and Exchange Participants

• DS-158 Requests extended contact information about you, people we can contact in an emergency (i.e., family, friends) and your work experience. Please print this form and complete manually at http://www.state.gov/documents/organization/79965.pdf
  
  
• I-20 or DS-2019 – provided by your school or exchange program.
  
  
• I-901 SEVIS Fee Receipt - All F, J, or M visa categories must pay an initial fee to register with the Student and Exchange Visitor Information System (SEVIS).  Please visit http://www.ice.gov/sevis/i901/index.htm for information on paying the fee online, through Western Union or normal mail.  All F, J, amd M visa applicants must present their I-901 SEVIS fee receipt at the time of their visa interview.
  

4)  Additional Forms for Professional Workers

Valid Work Petition approved by the U.S. Department of Homeland Security (DHS).  This should be provided by your employer or DHS.
