Visa Forms

NOTICE:  Applications must be completed and signed accordingly. If applications are incomplete due to forms missing, no payment, wrong photo, questions unanswered, or no signature,  you will be asked to schedule another appointment.  Paper application forms will no longer be provided on the day of your interview.

All applicants must present the following forms at the interview:

  • DS156 and DS157   
    • All questions must be answered
    • DS156 must be completed online, first, then click continue at the end to print three pages
    • Sign and date your DS156 
    • DS157 only must be completed by everyone over the age of 14
    • DS157 only can be completed by hand (manually)

All Students and Exchange Participants must complete an additional form

  • DS158 
    • all questions must be answered accordingly.
    • Please sign and date your application
  • I-20 or DS 2019 
    • signed and dated    
  • I-901 SEVIS Fee Receipt - http://www.ice.gov/sevis/i901/index.htm   
    • All F, J, or M visa categories must pay an initial fee to register with the Student and Exchange Visitor Information System (SEVIS).  Please visit  for information on paying the fee online, through Western Union or normal mail.  All F, J, amd M visa applicants must present their I-901 SEVIS fee receipt at the time of their visa interview.

Additional Forms for Professional Workers

Valid Work Petition approved by the U.S. Department of Homeland Security (DHS).  This should be provided by your employer or DHS.
