Replacing a Lost, Stolen or Mutilated Passport

If your U.S. passport has been lost or stolen, you should immediately inform the Consular Section at the U.S. Embassy in Doha by emailing ACSConsularDoha@state.gov or by calling +974 496 6000 ext. 6117 and leave a message.  If an emergency, please dial +974 496 6000 and hold for an operator or ask to speak to the U.S. Embassy Duty Officer after hours.

The American Citizen Services Unit can replace lost, stolen or mutilated passports upon verification of your identity and your citizenship.  If your passport has been lost or stolen, please be prepared to provide proof of your identity and citizenship as indicated in First-Time and Minor Passports.

Applications to replace lost, stolen or damaged passport are accepted by appointment during routine service hours.  However, if your passport has been lost, stolen or damaged and you need to travel for a life-or-death emergency, walk-ins will be accepted, subject to approval by a consular officer.  Please contact the ACS Unit in advance to inform us of your need for an emergency appointment.

PLEASE NOTE:  Your appointment refers to the time you should arrive in the Consular Section, including anticipating drive time and security screening.  Please take a number once you arrive in the Consular Section.  Your appointment time does not refer to the time you will be seen by U.S. Embassy staff.

Passport application forms can be obtained through https://pptform.state.gov.  Please do not sign the form until present before the consular officer.  We will not accept hand-written or hand-filled applications or offer blank paper applications.

REQUIREMENTS:

  • Passport Application Form(s) - https://pptform.state.gov.
  • Qatari Police Report, translated to English, if lost or stolen
     
  • Current U.S. Passport (if in your possession)

  • Photographs: Two (2) identical, recent photographs taken full face, with a white background, measuring 5 cm by 5 cm (2 in x 2 in). Please ask the photographer for photos for a U.S. passport, otherwise you may be given the wrong format.

  • $100 fee to replace a lost, stolen or damaged passport.  Fees are payable by major credit card or cash (U.S. dollars or Qatari Riyals). Exact change is preferred for cash payments. Personal checks are not accepted.
