Replacing a Lost, Stolen or Mutilated Passport

If you believe that your U.S. passport has been lost or stolen, you should immediately inform the Consular Section at the U.S. Embassy in Doha by emailing ConsularDoha@state.gov or by calling +974 488-4101 ext. 6117 and leave a message.  If an emergency, please dial +974 488 4101 and hold for an operator.

The American Citizen Services Unit can replace lost, stolen or mutilated passports upon verification of your identity and your citizenship.

To apply for a replacement passport, you must appear personally at the Consular Section with the following items:

  1. A police report substantiating loss or theft and translated to English.
  2. Form DS-64 (can be viewed using Adobe Reader which can be downloaded from http://www.adobe.com/products/acrobat/readstep2.html) Statement Regarding Lost or Stolen Passports (available at our office)
  3. Form DS-11 (view this form with Adobe Reader which can be downloaded from http://www.adobe.com/products/acrobat/readstep2.html) Please do not sign this form until asked to do so at the Consulate
  4. All available identification such as: birth certificate, driver's license, Identification card, or expired U.S. passport. If you have no current identification with you, you should bring along an American friend or relative with his or her passport to vouch for you and or photocopy of the missing passport.
  5. Two(2) identical, recent photographs taken full face, with a white background, measuring 5 cm by 5 cm (2 in x 2 in). Please ask the photographer for photos for a U.S. passport, otherwise you may be given the wrong format. See photo requirements for more information.
  6. Your Social Security number
  7. $100 Application and processing fee ($85 for minors under the age of 16). Fees are payable in cash (US dollars or Qatari Riyals). Exact change is preferred for cash payments. Personal Checks are not accepted. 

Minors under the age of 16

Both parents must appear together with the child and sign the application form in the presence of a Consular Officer. In the case where only one parent is able to appear, he or she must submit a notarized Statement of Consent from the absent parent, if living, authorizing passport issuance for the child. 

Parents or legal guardians must present their passports, along with a legible photocopy of the child's birth certificate.

Children 16 years or older must sign their own application form. 

Please be advised that once a passport is reported lost or stolen it cannot be used for travel. If found, all passports previously thought lost or stolen must be brought in to the embassy for cancellation.

When traveling, please remember to carry a photocopy of your passport separately from your passport. In the event of loss or theft, the copy will facilitate issuance of an emergency passport.
