Additional Passport Pages
All additional passport pages require, at a minimum:
1.) An appointment
2.) A completed application - that has been filled out online and printed.
Applications for additional passport pages and any passport amendments are accepted by appointment only during routine service hours. To schedule an appointment, please visit our appointment system for Americans Citizens Services at:

You must fill out and print, online, an application for visa pages before your appointment. If you do not have a compelted form you will be asked to leave and to return at a later date with a compeleted form.
PLEASE NOTE: Your appointment refers to the time you should arrive in the Consular Section, including anticipating drive time and security screening. Please take a number once you arrive in the Consular Section. Your appointment time does not refer to the time you will be seen by U.S. Embassy staff.
Forms for additional passport pages can only be obtained online at https://pptform.state.gov. We will not accept any hand-filled forms and will not provide blank application forms. If you have not scheduled an appointment or completed the form online, you will be required to return to the U.S. Embassy on subsequent business day.
Applications for adding visa pages will be taken and returned, generally, the same day. However, because of staffing, demand loads or downed computer service there may be a wait of up to 24 hours. There is no fee for this service. Please make sure that your passport is in good condition before having visa pages added to it.
The application form must be completed and signed. All persons over the age of 16 may sign the application and send the form along with their passport with someone else if they do not wish to come to the Embassy themselves.
Minors under the age of 16:
Both parents must appear together with proper identification and sign the application form in the presence of a Consular Officer with either a birth certificate or a Consular Report of Birth Abroad (CRBA) for the child.
OR
If only one parent can appear (because the other cannot appear due to travel abroad or other exigent circumstances), the present parent must sign the application in the presence of a Consular Officer, AND submit a notarized Statement of Consent from the absent parent authorizing passport issuance for the child.
Correcting Information in your Passport
To amend your passport outside the U.S., you must renew your passport. However, in certain circumstances, you may be required to provide additional information and may not be charged the passport fee.
