How to Apply for A First-Time or Minors Passport
We no longer accept hand-written / filled applications or offer blank paper applications. Passport application forms can be obtained through https://pptform.state.gov. Please do not sign the form until present before the consular officer.
To apply for a first-time or minors passport, all applicants must apply in person, including minor children. Parents must appear together with and execute the application for a child under 16 years of age.
When applying for a child under age 16, parents or legal guardians must submit a legible photocopy of their U.S. passports, along with the child's birth certificate or a certified true copy by the authority who issued the birth certificate. Both parents must be present to sign the application for all minors under age 16.
It takes approximately 10 business days to receive your new tourist passport, so please plan accordingly. Official and Diplomatic passports can take much longer, sometimes as long as two months.

PLEASE NOTE: Your appointment refers to the time you should arrive in the Consular Section, including anticipating drive time and security screening. Please take a number once you arrive in the Consular Section. Your appointment time does not refer to the time you will be seen by U.S. Embassy staff.

Photographs: Two (2) identical, recent photographs taken full face, with a white background, measuring 5 cm by 5 cm (2 in x 2 in). Please ask the photographer for photos for a U.S. passport, otherwise you may be given the wrong format.

$85 fee for minors under the age of 16. $100 fee for adult applicants applying for their first passport. Fees are payable by major credit card or cash (U.S. dollars or Qatari Riyals). Exact change is preferred for cash payments. Personal checks are not accepted.
One Parent Custody and Statement of Consent
When one parent is currently out of Qatar or resides in another country, one parent can appear with the child, sign the passport application form in front of the consular officer and submit the second parent's notarized Statement of Consent form authorizing passport issuance for the child.
One parent may obtain a passport for their child if the parent submits primary evidence of sole authority to apply (such as one of the following):
- Child's certified U.S. or foreign birth certificate (with translation, if necessary) listing only applying parent; or
- Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only applying parent; or
- Court order granting sole custody to the applying parent (unless child's travel is restricted by that order); or
- Adoption decree (if applying parents is sole adopting parent); or
- Court order specifically permitting applying parent's or guardian's travel with the child; or
- Judicial declaration of incompetence of non-applying parent; or
- Death certificate of non-applying parent.
Proof of US Citizenship
Applicants born in the United States: Submit an original or certified copy of a U.S. birth certificate. A birth certificate must include your given name and surname, date and place of birth, date the birth record was filed, and seal or other certification of the official custodian of such records. A record filed more than 1 year after the birth is acceptable if it is supported by evidence described in the next paragraph.
If no birth record exists, submit registrar's notice to that effect. Also submit an early baptismal or circumcision certificate, hospital birth record, early census, school, or family bible records, newspaper or insurance files, or notarized affidavits of persons having knowledge of your birth (preferably with at least one record listed above). Evidence should include your given name and surname, date and place of birth, and seal or other certification of office (if customary) and signature of issuing official.
Military personnel without access to original birth certificate may submit a faxed, notarized copy of their birth certificate. Please refer to http://travel.state.gov/passport/get/first/first_830.html for more information.
Applicants born outside the United States must submit a Certificate of Naturalization, Certificate of Citizenship, Report of Birth Abroad, or other evidence described below.
If you claim citizenship through naturalization of parent(s), submit the certificate(s) of Naturalization of your parent(s), your foreign birth certificate, and proof of your admission to the United States for permanent residence.
If you claim citizenship through birth abroad to US citizen parent(s), please submit a Consular Report of Birth (Form FS-240) or Certification of Birth (Form DS-1350 or FS-545). You may also submit your foreign birth certificate, parents' marriage certificate, proof of citizenship of your parent(s) and affidavit of U.S. citizens parent(s) showing all periods and places of residence/physical presence in the United States and abroad before your birth.
Child Citizenship Act
The Child Citizenship Act of 2000 (CCA) went into effect on February 27, 2001. The CCA grants automatic or expeditious naturalization to the minor children of U.S. citizens abroad. Step-children do not qualify to acquire U.S. citizenship through their U.S. citizen step-parent.
Proof of Identity
You must establish your identity to the Consular officer's satisfaction. You may submit a government issued document such as the following contained your signature and physical description or photograph that is a good likeness of you; certificate of Naturalization or of Citizenship; driver's license (not temporary or learner's license); or government (Federal, State, Municipal) identification card or pass. Temporary or altered documents are not acceptable.
If you cannot prove your identity as stated above, you must appear with an identifying witness who is a U.S. citizen or permanent resident alien who has known you for at least 2 years. Your witness must prove his or her identity and complete and sign an Affidavit of Identifying Witness (Form DSP-71) before the Consular Officer. You must also submit some identification of your own.
Two identical and recent photographs must accompany the application. Photos must be taken full face with a white background measuring 5 cm by 5 cm (2 in. x 2 in.). Please tell the photographer that the photos are specifically for a U.S. passport. (Photo specifications)
